Becoming a new patient is simple – just choose one of the following options to get started!
During the new patient process, our intake coordinator will gather basic patient data and insurance information, then schedule an appointment with the clinician most suited for your needs.
Once you have an appointment scheduled, you may download the New Patient Forms required for all new MindPath Care Centers clients to bring to your first appointment. The data on this form will enable us to file your insurance claims on your behalf. Simply download the PDF file onto your computer, complete at your leisure, and bring with you to your first appointment, or complete the forms in the office 15 minutes before your appointment time. You may need to download Adobe Acrobat Reader to access these forms.
We verify your benefits and file all claims for our clients. In most cases, only co-pays and deductibles are due at the time of service. We will also file to smaller carve-outs and offer out of network benefits for most plans. Self-pay rates are available for uninsured clients who qualify. Payment plans are also available.
